Tell Your Story
Become a Faith Community Nurse, Health Minister or Community Health Advocate to serve your faith community or geographic community.
Once you graduate from this course, you have the option to be listed in our directory of volunteers and be invited to ongoing continuing education events (can be used towards nursing license requirements!)
If you have already taken a Foundations course and are seeking to become a part of Health Ministries Network, please contact us. We just need a copy of your certificate, a quick background check and enrollment form and you are ready to go!

You don’t need the “Foundations Training” to participate in special events such as health fairs, clinic invitations, etc. We are very happy to find a place for individuals who would like to help with non-medical support tasks, as needed. Your name will not be included in the directory.

If you have been assisted by our volunteers and programs and would like to share your story we would love to publish your personal account in our newsletter and other correspondence with sponsors and those who are interested in our mission.

Other Ways to Be Involved with Us
Contact a Faith Community Nurse, Health Minister or Community Health Advocate in your area.
See who is listed in our directory of volunteers. If you are outside of our four-county service area, please contact the director.
Connect with Transitional Care Support.
Please work with your patient’s primary care team/discharge team to coordinate transitional care. If there is a Faith Community Nurse in your faith community/geographic community who can provide assistance, they will connect first with medical providers and administration to coordinate care, if possible.
Serve on our Board of Directors.
