ADVANCE CARE PLANNING
Planning Ahead for Peace of Mind
Making your own choices and sharing them with your family and your medical providers is a huge gift to others, and is a way of taking charge of your own life.
Advance care planning (ACP) is making decisions about the healthcare you would want to receive if you are unable to speak for yourself. These are your decisions based on your personal values, preferences, and discussions with your loved ones.
ADDITIONAL RESOURCES
NWRC Workshops
Peace Health eliminated their Advance Care Planning Department. Advanced Care Planning has been transitioned to Peace Primary Care Providers.
ADVANCED CARE PLANNING INCLUDES:
Getting information on the types of life-sustaining treatments that are available.
Deciding what types of treatment you would or would not want in the case of a life-limiting illness or accident.
Choosing who you would have speak for you –if you are unable to speak for yourself.
Writing down your choices, having these witnessed and notarized, and sharing them with those concerned (family, medical facilities, providers, etc.)
