All HMN volunteers are encouraged to submit their 2018 service hours now.
Choose one month from this year to report your own HMN volunteer hours. The online form is 9 questions long and takes about 5 minutes to complete. Submissions received by 4 pm on Tuesday, December 4 will be considered for awards.
Awards will be presented at the upcoming:
Spirit of Giving: Annual HMN Recognition Lunch and Fundraiser

New: Snowflake Awards
All HMN faith community nurses and health ministers who submit their hours will receive a lovely ceramic snowflake. Must be present at Spirit of Giving: Annual HMN Recognition Lunch to claim your ornament.
Even the strongest blizzards start with a single snowflake.
The Angel Awards are back
We’ll be presenting six beautiful Australian crystal angel pins to those who report the most volunteer hours for:
Routine visits/wellness checks
Blood pressure clinics
Disease specific consultations / referrals
Palliative care visits
Transitional care contacts
Continuing education/ health care meetings attended
Clergy Champion
As part of our angel awards, we are also soliciting nominations for a Clergy Champion who has supported health ministry in their congregation. When you fill out the survey, please consider nominating a leader from your church who has championed health ministry!
